The assessment describes the cell you have, not the one you designed.
When to update

How to keep it accurate
- Record the revision history with dates and reasons.
- Involve the people who operate and maintain the cell.
- Re-verify affected safety functions after the update.
Risk assessment is a specialist task. Involve a competent person for any significant change.
